8th Grade Dance Permission Slip Form.
Lakeside Promotion Dance – Friday, May 31 6:00 – 8:00 PM - Students MUST have a ticket in order to attend the dance.
This dance is for current 8th grade Lakeside students only. Admission will be provided by ASB at no charge. Students must bring the permission slip found on Lakeside’s website to the dance. Suggested attire: dressy and special occasion but NOT formal. Students must arrive at the dance no later than 6:30PM and may not leave unless picked up by a parent. Lakeside Promotion Ceremony – Friday, June 7 7:30 – 9:00AM
On this day 8th graders and their parents are invited to enjoy light refreshments outside the lunch area at 7:30 A.M. Please note: due to the size of this 8th grade class, this program is intended for our 8th graders and parents only. Eighth grade students will report to their advisor behind the 300 building at 8:00 A.M. and the ceremony will take place from 8:15 A.M.– 9:00 A.M. Note: Students will be leaving immediately after the above activities for the trip to Knott's Berry Farm, so they should dress accordingly but consistent with our Lakeside Dress Code.
Knott’s Berry Farm Field Trip – 9:30 AM – Approximately 5:45 PM
Tickets for this trip cost $45.00 ($35.00 for admission ticket and $10.00 for transportation). Students MUST be in good standing academically and behaviorally to attend this field trip. Permission slips have already been collected. We can only accept exact cash at this point in the year. No checks will be accepted.
STUDENTS MUST TAKE THE SCHOOL BUS TO KNOTT'S BERRY FARM AND RETURN TO LAKESIDE ON THE SCHOOL BUS. Students who are NOT going to Knott’s must sign out in the front office.
Please note: ALL Lakeside library materials, books and textbooks must be returned to the Library Media Center IMMEDIATELY. All payments for fines must be in cash or CHECKS PAYABLE TO IUSD. All unpaid fines will follow the student through IUSD until the balance is paid.
